Dr. Steven Stradley, Dr. Elizabeth Hagerty, and Dr. Katy Stradley are contracted providers with Anthem 300 and Delta Dental Premier. We can file with all insurances unless network participation is required.
Please provide a copy of all insurance cards and any medication lists upon arrival.
Registration, HIPAA, and financial responsibility forms are filled out in office and are required.
All co-pays and deductibles are due at the time of service. Payment options are available through Care Credit with eligibility.
